Legacy Senior Living is owned and operated by Legacy Senior Care CEO Novella Lanzanas, a Registered Nurse with over 20 years of combined experience in adult bedside nursing, home care, as well as disease and population health management.  Legacy Senior Living is here to assist you with both long-term or respite solutions, providing excellent and professionally trained staff for live-in care.

Our Services

  • 24 Hour care, assistance and supervision provided by highly trained staff to ensure constant safety

  • Room & Board, Housekeeping & Laundry, Meals & Snacks, and Recreational / Social Activities  

  • Private and semi-private rooms available 

  • Three meals and snacks served daily catering to individual nutritional needs and preferences 

  • Medication review and reconciliation conducted by a Registered Nurse upon Admission to the facility, as well as at every change in treatment plan

  • Cable TV,  internet and phone services available for residents and staff

  • Various recreational activities conducted for continued physical, mental and social stimulation

  • Services of Visiting Physicians, RNs, Physical Therapists, Occupational Therapists or Speech Therapists available to residents on an as needed basis

  • Registered Nurse on call

FEES FOR CARE

 

Monthly fees are charged based on level of care needed by prospective resident and is determined after needs assessment has been completed.

 

ESSENTIAL CARE

 

  • Resident needing minimal assistance with transfers and ambulation with the use of walker or cane

  • Resident requiring minimal assistance with eating, hygiene, bathing, toileting, dressing, and  grooming

  • Individually Scheduled Medication Administration by highly trained Staff

  • Weight Monitoring with frequency depending on medical condition  

  • Residents with mild dementia

 

INTERMEDIATE CARE

 

  • Residents needing moderate assistance with hygiene, bathing, toileting, dressing and grooming 

  • Residents with bladder incontinence requiring regular diaper change or with an indwelling foley catheter

  • Residents requiring one on one assistance with feeding

  • Residents requiring frequent verbal instructions and reminders on Activities of Daily Living

  • Residents on Aspiration Precaution requiring a special diet (ex. pureed diet) and crushed medications

  • Residents with moderate dementia

 

ADVANCED CARE

 

  • Resident requiring total care with hygiene, bathing, toileting, dressing, grooming and feeding

  • Residents with bowel and bladder incontinence requiring frequent diaper and linen change

  • Residents with severe dementia with evidence of wandering and/or sundowning
